PLEASE NOTE - A SEPARATE REGISTRATION FORM & FEE IS REQUIRED FOR EACH CONFERENCE ATTENDEE. IF REGISTERING MULTIPLE PEOPLE AT ONCE ONLINE, PLEASE PROVIDE NAMES FOR EACH PERSON IN THE NOTES SECTION OF CHECK-OUT.  

A limited number of meals-only packages are available for the spouses and guests of a full conference attendee. Meals-only registrations are also available through our secure online store or by calling us.  When selecting the meals option for a guest, please include guest/spouse name in the notes section of check-out.

By registering for the conference, I give permission for the free use of my name and photo in any media account of this event. I also certify that I am 21 years of age or older.

Cancellation policy: For a refund, less a $100 administrative charge per person, send written notice by April 30, 2010. Refund requests received after April 30, 2010 will not be refunded. All refund requests will be processed post-conference.
